To request patient information, please complete the Authorization for Release of Health Information form. Completed forms must be delivered in person, by postal mail, or by fax — requests cannot be accepted by email.

White Oak EMS is available to provide EMS standby coverage for community events, sporting events, and large gatherings within our service area. Submitting a request early helps us schedule appropriate staffing.
